Having multiple problems: horizontal lines, channel lock code, GuidePlus Menu.

My JVC tv (AV-36D800) began having horizontal lines all throughout an all blue screen and looks as if it is stretched out bigger than the screen. Also, a CHANNEL LOCK CODE keeps appearing. After about 3 1/2 minutes the GuidePlus Menu appears and then you can't access channels or menu or anything. We don't have cable or an antennae but watch dvds and videos.
