Equipment for Broadcasting

I am looking to take my web radio show live. I would like to have a phone line to enable call ins via an 800 number. I would also like the ability to add commercial ads throughout the program, which will be 1 hour weekly. Can anyone tell me what kind of hardware and software I would need to do such a thing? Thanks.
