I have seen this fault a number of times in my lab. The CCD imager is at fault, one or more of the tiny connecting wires comes adrift from the imager substrate. These wires are 10 microns wide (a human hair is around 30), and are part of the hermetically sealed CCD package. Repair involves replacing the CCD assembly & re-calibrating the electronics to match the replacement imager. Adjustments are made using software.
