How to make custom video cables that connect VGA to s-video or RCA terminals

I was wondering how to make custom cables with different connections on each end, particularly a cable with a 15-pin VGA connection on one end and an s-video connection on the other, or RCA. Can you make it from coax? What are the specific steps/wire connections to make a good quality cable with these terminals?
